There are two possible scenarios why you are receiving this error:
User not active
User name or password incorrect- Password change prompts failing
Legacy username formats no longer working
To resolve this follow the steps below.

âNote: If you or your administrator don't have access to User Profiles, please ask a question in the MyAccess Portal Digital Assistant and ask to "Speak to a human" to be put through directly to our support team.
Activate the user
From Proclaim open User Profiles.
Select the user who's unable to login.
Click the MAIN tab.
Check if the Live User check box is selected.
If the check box is already selected, proceed to the Incorrect user name or password section.
Click the UPDATE button.
Tick the Live User check box.
Click the SAVE button.
The user now activated, try to log back into Proclaim. If the error message reappears it maybe that the user name and password are incorrect, if so please proceed to the next section.

Check user name
From Proclaim open User Profiles.
Select the user who's unable to login.
Click the MAIN tab.
Check User Login name used is correct.5. If the username is not accepted, try using your initials as Proclaim usernames are typically the user's initials. If you previously used a legacy format (e.g., DOMAIN\Firstname.Surname), try entering your initials instead.
If the User Login name is correct please, try logging in again checking that the details are being entered correctly. If the error message still appears, you'll need to reset the password.Double-check for typos and ensure that Caps Lock is off when entering your credentials.
Reset password
From Proclaim open User Profiles.
Select the user who's unable to login.
Click the MAIN tab.
Click the UPDATE button.
Enter a new password in the Password field.
Click the SAVE button.7. Ensure that the current password is entered correctly before setting a new one. Make sure both new password entries match exactly. If you cannot remember your original password, contact an administrator to reset it.
You will now be able to login using the user name and password.If login issues persist, double-check your entries for typos, ensure Caps Lock is off, and contact your administrator for further assistance.
